Required Immunizations
Immediately upon enrollment in the district, the student’s parent/guardian must provide proof of immunization status with a Certificate of Immunization Status (CIS), approved by the Washington Department of Health in accordance with 28A.210 RCW, the Washington State Board of Health 246-105 WAC and district policy 3413.

Obtain Immunization Records
Obtain Your Child's Immunization Records from Washington State
MyIR is a consumer access portal that parents and individuals can use to view their own and their children's immunization records securely online. Once registered for MyIR, they can access their immunization records any time they are needed. They can also print the Certificate of Immunization Status (PDF) for school and child care entry.
How to Sign Up
Step 1: Go to MyIR to begin the sign-up process. Choose the Auto Match option and if the system finds an exact match, MyIR will send a verification code by either text or phone call. If an exact match is not found, MyIR will direct the user to Step 2.
Step 2: Fill out an Authorization to Release Immunization Records (PDF) or an Autorización para entregar documentos de vacunaciones (PDF) and check the box indicating the user wants to access their records online via MyIR. Mail, fax, or e-mail the form to:

Once the signed form is received, Office of Immunization and Child Profile staff will register the person in MyIR and send a temporary PIN with instructions on what to do next.
