Employee Leave

A Yelm Community Schools leave request form must be submitted when an employee is requesting leave for medical, disability, family medical, or maternity/paternity. After completing the form, please submit it to Ashley Lindsay in Payroll.

Leave Request Form

This leave allows you time off to grieve and manage affairs after losing a loved one. You can submit the request through Ready Sub, Skyward, or a time sheet, depending on your position. Make sure to state your relationship to the deceased when asking for this leave. More detailed information is available in your Collective Bargaining Agreement or in School Board Policy in the 5000: Personnel section.

It is the intent of the District to foster a safe and supportive community for all employees, students, and visitors. Employees who experience the effects of domestic violence, stalking, or sexual assault are encouraged to utilize job-protected leave from the workplace in order to seek and obtain assistance. Please reference your Collective Bargaining Agreement or the School Board Policy in the 5000: Personnel section. For specifics on jury duty leave. You will be required to submit documentation of jury duty service to Payroll.

This is a request for time away from work to fulfill duties and obligations for certain types of active or inactive duty in the National Guard or as a Reservist of the Armed Forces. All employees who are members of any of the United States uniformed services are eligible for military duty leave.

School district employees accrue personal leave. Refer to your Collective Bargaining Agreement or the School Board Policy in the 5000: Personnel section for the number of days accrued, required advance notice when requesting use, and how and when they can be used.

You can use your sick leave for your own illness, injury, or emergencies. When this type of leave is available, approved time off taken will be paid at your regular salary amount. Sick leave can be used for your immediate family and is more fully described in your Collective Bargaining Agreement or in School Board Policy in the 5000: Personnel section.

Eligible employees can take up to 12 weeks of unpaid leave each year for family or medical reasons. The FMLA entitles eligible employees to take unpaid (or can utilize accrued sick or vacation leave), job-protected leave for qualifying family and medical reasons and requires
continuation of their group health benefits under the same terms and conditions as if the employee had not taken leave.

This Washington State program provides partial wage replacement for employees taking time off for family or medical reasons, such as caring for a newborn or a seriously ill family member. You need to inform your employer at least 30 days in advance if possible. Applications and approval are handled through the Employment Security Department.

WA PFML, you must return to work by:
The first scheduled workday after your leave ends, or After 16 weeks of leave in a year (18 weeks if related to pregnancy complications). If you
don’t return on time and don’t have a written agreement to extend your leave, your employer doesn’t have to restore your job. More detailed information is available on the PFML website @ WA PFML Job Protection

The District's Shared Leave Program allows other employees within the District to donate leave to you for a serious health condition, care for a parent or child with a serious health condition, and Parental Leave. If you believe your medical condition falls within the parameters of Shared Leave eligibility, please complete a Shared Leave Application.

To donate leave to someone who has been approved for Shared Leave, please complete
the Shared Leave Donation form.

Work injuries happen. When they do, you need fast help and fair treatment. Impact of Leave Without Pay on Benefits and Service Credits
Taking a leave of absence without pay may affect various aspects of your employment benefits and service credits. Consider the following potential impacts:

Service Credits with DRS (Department of Retirement Systems)
While on a leave of absence without pay, you may not accrue service credits towards your retirement benefits. This could impact your retirement date and the amount of your pension.

Eligibility for Benefits through SEBB (School Employees Benefits Board)
Your eligibility for SEBB benefits, such as health, dental, and vision insurance, may be affected during your unpaid leave. You may need to arrange for self-pay to maintain these benefits during your leave period. Please contact the Benefits Office for specific details and options.
