Athletic Eligibility PacketsFormsThe Eligibilty Form must list all information, including medical insurance, to be considered complete. Signing this form indicates you have read and understand all paperwork associated with participation in YCS sports, including the Yelm Community Schools Athletic and Activities Code. If you play a 2nd or 3rd sport within the school year, you will only need to fill out a new Emergency Card and a Risk Letter for the sport you will be participating in. You must provide a copy of your last semester grades. This information can be printed via student logins on Skyward. Your initial sports eligibility may be affected by your previous semester's grades. Each athlete must have a current physical to participate. NO EXCEPTIONS! Physicals are valid for 13 months from the date of issue. It is the athlete's responsibility to be aware of the expiration date of his or her physical. Please submit paperwork in advance. Athletic clearance requires at least 24 hours. PLEASE NOTE: Incomplete packets cannot be accepted.
